OC Health Craven Exemption form
(for-profit exemption) – By signing this document, you confirm your business is participating for marketing purposes, not to sell goods.
Restaurant Information Form
This form lets us know what we still require from you (your needs for type of container (plates, trays, cups, etc, insurance certs, power needs, what you will be serving, etc.). It will also be sent to you via email about three weeks before The Taste.
