OC Health Craven Exemption form
(for-profit exemption) – This is required by Orange County Environmental health. This document confirms your business is participating for marketing purposes, not to sell goods. The very first line is where you put the name of your business
Restaurant Information Form
This form allows you o tell us your needs, such as type of container (plates, trays, cups, etc,) insurance certs, power needs, what you will be serving, etc.). It will also be sent to you via email about three weeks before the event.
